What the D4 medical covers
The DVLA sets a higher medical standard for Group 2 licences than it does for cars, because you will be in charge of a vehicle weighing up to 44 tonnes or carrying passengers. The clinician works through the official D4 form section by section: eyesight (including visual field and colour recognition), blood pressure, heart and circulatory history, diabetes and how it is managed, neurological conditions including epilepsy and blackouts, sleep apnoea and daytime sleepiness, mental health, alcohol and drug history, and any prescribed medication that could affect driving.
Nothing on the list is designed to catch you out. Most drivers pass first time. The common hold-ups are blood pressure that reads high on the day, glasses that are out of date, or a diabetic driver arriving without recent monitoring records. If you know one of those applies to you, sort it before the appointment and the whole process stays quick.
You keep the completed D4 and send it with your D2 application form to the DVLA. Turnaround from the DVLA is usually two to three weeks, so book the medical as early as you can — it is the single most common reason a training course start date slips.

What is an HGV (D4) medical?
A D4 medical is the DVLA health assessment every Group 2 licence applicant must pass before a provisional HGV, LGV, PCV or bus entitlement is issued. A clinician checks your vision, blood pressure, heart, neurological history, diabetes control, sleep-apnoea risk, mental health and any medication you take, then completes the DVLA D4 form.
Who needs one?
Anyone applying for a first Class 1 (C+E), Class 2 (C), C1 7.5-tonne, D or D1 licence, plus existing licence holders renewing at 45 and then every five years, and annually from age 65. Taxi and private-hire drivers often need an equivalent medical for their council licence.
How long does the appointment take?
Around 20 to 30 minutes. You leave with your completed D4 form the same day — there is no waiting on a GP surgery to post it out.
What should I bring?
Photo ID, your current driving licence, glasses or contact lenses if you wear them, a list of any medication you take, and details of your GP practice. If you have diabetes, bring your recent HbA1c readings and monitoring records.
What if I fail part of the medical?
Most issues are fixable. High blood pressure on the day, uncorrected vision or missing diabetic records are the common causes. The clinician will tell you exactly what the DVLA needs and you can be re-checked once it is sorted.
Do I need the medical before I start training?
Yes. Your provisional Group 2 entitlement is issued off the back of the D4, and you need that entitlement before you can sit the theory test or drive one of our lorries. Book the medical first, then the theory, then your practical course.
